No noto nada raro ahi. Quizás habría que revisar la totalidad del log a ver si hay mensajes de error en las líneas que ese filtrado no devolvió. De no haber errores, haciendo (como superusuario) un dd y luego un fdisk sobre /dev/sdb deberías resolver. Si no funciona, puede que la memoria esté dañada (lo cual sería muy raro si no hay otros mensaje de error).

Bueno, ahi lo que dice básicamente es que hubo errores de entrada/salida al intentar acceder al dispositivo.
En mi opinión, lo más probable es que la memoria esté dañada. Solo por curiosidad, ejecuta este comando a ver que muestra (debes haber instalado lsusb)
